Hospital to Home Program
If you’re undergoing a complex surgery, you might benefit from some extra support. That’s why we offer various programs to assist with recovery and management of any ongoing health conditions. The programs are managed and delivered by a network of registered nurses with the support of other health professionals like physiotherapists.
Diabetes Management Program
The Diabetes Management Program provides personalised information, support and education if you’re living with diabetes. This program is led by a credentialed Diabetes Educator and supported by nursing staff.
