Here are a few of our programs:

Hospital to Home Program

If you’re undergoing a complex surgery, you might benefit from some extra support. That’s why we offer various programs to assist with recovery and management of any ongoing health conditions. The programs are managed and delivered by a network of registered nurses with the support of other health professionals like physiotherapists.

Newborn Support Program

The Newborn Support Program provides ongoing support, from the time you find out you’re pregnant through to the first 8 weeks of your baby’s life. You’ll even have access to an experienced midwife.

Diabetes Management Program

The Diabetes Management Program provides personalised information, support and education if you’re living with diabetes. This program is led by a credentialed Diabetes Educator and supported by nursing staff.
