If you’re going to hospital and undergoing a complex procedure or have a serious health condition, you might benefit from some extra care. That’s why we offer various support and recovery programs under Health Partners Hospital to Home.
Our programs are managed and delivered by registered nurses with the support of other health professionals like physiotherapists.

This program is designed to help alleviate the stress and confusion of going to hospital. A team of registered nurses will contact you before your admission, chat through what’s to come and provide support and guidance while you’re in hospital. They’ll then work directly with your in-hospital medical team to get you back to the comfort of your home.
Everybody’s health situation is different. We’ll assess your individual situation and if extra care or support is required, refer you for a personalised care program. These programs are available at no additional cost to members with Health Partners Hospital Cover. Contact us to learn more.
This program is designed to help make your transition from hospital to home easier by providing hospital-like care at home. If you’re clinically able and your doctor agrees, you might be able to leave hospital early and finish your recovery in the comfort of your own home.
A personalised care plan will be developed and reviewed by the hospital before you’re discharged. Your plan will then be managed and coordinated by registered nurses at all times to provide extra care and support such as wound management, intravenous antibiotics, medication management, pain relief and other specialised nursing services.
A great alternative to in-hospital rehabilitation. You’ll be given a tailored, comprehensive rehab program that’s managed by registered nurses and delivered by allied health professionals such as physiotherapists in the comfort of your own home.

Studies have shown that in-home rehabilitation can work as well or better than in-hospital rehabilitation for certain procedures.1
All cases are individually assessed and programs are developed appropriate to a member’s individual needs. Programs and services will vary by provider in each State.
