Dental
Health Partners Dental
At Health Partners Dental you receive:
- 60% benefit or discount for general and major dental services
- 60% benefits or discount for specialist treatment
If you reach limit at Health Partners Dental you receive a further unlimited 40% benefit
Other Dental
A schedule of benefits apply for general, major and specialist dental services provided by dentists other than Health Partners professionals. Please call our Member Services for a quote.
Dental Limits
Total combined Dental limit is $500.
Orthodontic
60% benefit available for all orthodontic treatment:
- Including appliances such as plates, guards and spacers
- Lifetime limit of $200 after 1 year or
- Lifetime limit of $1,000 after 2 years.
Optical
Health Partners Optical
At Health Partners Optical you receive:
- 60% benefit or discount for prescription glasses and contact lenses
- Save 30% on all sunglasses.
If you reach limit at Health Partners Optical you receive a further unlimited 40% benefit
Other Optical
At Other Optical you receive:
40% benefit for prescription glasses and contact lenses provided by optometrists other than Health Partners professionals.
Optical Limits
Benefits claimed at Health Partners Optical will count towards Other Optical limits.
Pharmacy
Health Partners Pharmacy
Exclusive pharmacy benefits are available at over 40 Health Partners participating pharmacies.
You pay only $16.45 or $32.90 for an extensive range of prescriptions - limit $500. Your payment is indexed to the Federal Government PBS and reviewed each year. Prices effective January 1 2009.
Special member offer:
- You receive a 20% saving on non-prescription items such as vitamins, cosmetics, gifts, toiletries and more - no limit
Physiotherapy
Health Partners Physiotherapy
Exclusive physiotherapy benefits are available for services conducted at any Health Partners participating physiotherapy practice.
You pay only:
- $15 initial and subsequent consultations
- $25 long consultation
- $30 lymphoedema treatment
- $25 home or hospital visit
- $5 hydrotherapy
- Limit $750.
Other Physiotherapy
The following benefits are available for services provided by Physiotherapists other than Health Partners participating professionals.
- $21.50 initial consultation
- $16 subsequent consultation
- $22.50 long consultation
- $28 prolonged consultation
- $23 lymphoedema treatment
- $6.50 hydrotherapy
- $19 home or hospital visit
- Up to $10 group treatment
- Limit $450.
Phsyiotherapy Limits
Total combined Physiotherapy limit is $750.
Naturopathy / Homoeopathy
Benefit:
- $26.40 initial consultation
- $20.40 subsequent consultation
- $28.80 long consultation
- $36 after-hours consultation
- Combined limit $350 for Chiropractic / Naturopathy / Acupuncture.
Acupuncture
Benefit:
- $27.60 initial consultation
- $21 subsequent consultation
- $30.60 long consultation
- $37.80 after-hours consultation
- Combined limit $350 for Chiropractic / Naturopathy / Acupuncture.
Chiropractic/Osteopathic
Benefit:
- $22 initial consultation
- $17 subsequent consultation
- $24 long consultation
- $29.50 after-hours consultation
- $29.50 home visits
- $35 after-hours home visit
- Radiology 60% of fee charged (in professional's rooms)
Combined limit $350 for Chiropractic / Naturopathy / Acupuncture.
