National Extras is especially for Health Partners members living in country South Australia and interstate. This cover provides you with the following higher benefits at many of your local health professionals outside the Adelaide metropolitan area.

Plus you can also take advantage of Health Partners professionals anywhere in South Australia  for even higher benefits. You can search for participating providers by using the Partner Location Search.

When accessing services within the Adelaide metropolitan area, you receive the same benefits and limits as Gold Extras.

Dental

A schedule of benefits applies for general, major and specialist services. Please call our Member Services for a quote (ensure you have your dentist's provider number, and the item number(s) and fees for your treatment handy).

• General dental – no limit
• Major dental – limit $1,100
• Specialist treatment – limit is the same as general and major dental.

Orthodontic

Benefit:

• 70% for orthodontic treatment provided by an orthodontist
• 50% for orthodontic treatment provided by a general dentist
• Includes appliances such as plates, guards and spacers.

Limit:

• Lifetime limit of $500 is available after 1 year, or
• Lifetime limit of $1,500 is available after 2 years, or
• Lifetime limit of $2,500 is available after 5 years.

Optical

Benefit:

• 55% for prescription glasses and contact lenses.
  Overall limit of $300 (this includes a limit for frames of $100).

Pharmacy

Pharmacy Connect

Exclusive benefits and discounts are available through Pharmacy Connect.

For an extensive range of prescriptions:

• You pay only $17.70 or $35.40.

Your payment is indexed to the Federal Government PBS and reviewed each year. Prices effective 1 January 2012.

Combined limit $600 for Pharmacy Connect and your local pharmacy.

Special Member offer:

• You receive a 20% saving on non–prescription items such as vitamins, cosmetics and more. Conditions apply.

Conditions: 20% discount is not available on some pharmacy prescriptions, agency lines and weight loss shakes/supplements (some pharmacies only).

• $20 per consultation for remedial or therapeutic massage

Psychology

Benefit:

Hypnotherapy

Speech Therapy

  Limit $400, $600 after 5 years of continuous membership.

Eye Therapy

Benefit:

• $25 per consultation
  Limit $250.

Occupational Therapy

Benefit:

• $50 initial consultation
• $40 subsequent consultation
• $15 group consultation
  Limit $350, $500 after 3 years of continuous membership.

RDNS

You receive 100% for already subsidised RDNS services.

Full Ambulance Cover

With our Ambulance Cover, you get real peace of mind because you are covered in full for ambulance costs, whether it's an emergency or not. Full cover includes:

• Emergency and non-emergency services
• Australia-wide coverage
• Road and air ambulance services
• Every service required on medical grounds (excludes clinic-car type transport)
  Limit $20,000.

Apparatus

You receive 85% of fee charged for the following apparatus:

• Asthmatic Spray Appliance – limit $130 every 3 years. Certain appliances can be purchased through Health Partners.
• Blood Glucose Monitoring Machines – limit $250 every 3 years. Members can purchase certain machines from Diabetes SA including 12-month membership of the association.
• Blood Pressure Machines – limit $100 every 3 years. Your limit is combined with Blood Glucose Monitoring Machines.
• Hearing Aids – limits based on years of continuous membership: 1–4 years – $800; 5–9 years – $900; 10+ years – $1,150. Maximum benefit limited to once every 5 years.
• Sleep Apnoea Machines – limit $1,500 every 5 years.
• CPAP Apparatus Support – limit $150 every 5 years when using a Health Partners participating provider.

A letter from your medical practitioner will be required for Asthmatic Spray Appliances, Blood Glucose Monitoring Machines and Blood Pressure Machines. We may request a supporting letter from your medical practitioner for all other apparatus benefits.

Health Management

• Online Health Profile – limit 4 per year. Access the Online Health Profile through Members Online.
• Quit Smoking – $100 benefit towards the cost of Nicotine Replacement Therapy when purchased through Pharmacy Connect or your local pharmacy. Available on completion of a Health Partners approved government Quit Smoking program. Lifetime limit $200.
• Weight Watchers – $50 benefit for first month of membership, plus $100 benefit upon achieving goal weight. Lifetime limit $150.
• Bowel Cancer Screening – $22 benefit towards testing fees for a home Bowel Cancer Screening test. Limit 1 per year.

• Within the Adelaide metropolitan area, benefits and limits are the same as Gold Extras.
• Benefits for services in the Adelaide metropolitan area are included in your non-Adelaide metropolitan limits.
